Description:

Lifestar Mental Wellness Center provides the following services:

Mental Health Services: individual, couples, family, group therapy.

Psychosocial rehabilitation program

Substance abuse: only outpatient programs - individual or groups

Case Management Services: assist in obtaining resources available in the community such as housing benefits, food stamps, economic benefits, donations, also assistance to apply for US citizenship, US residency, and other legal problems.

8:30am-5:30pm, Mon-Fri

Intake Process:

Contact by phone for more information, email at lifestar.wellness@gmail.com, or in person. Intake includes basic questions to assess needs and determine eligibility. A provider (therapist or case manager) is then assigned.

Program Fees:

Accepts Medicaid, Medicare, most major insurance plans, Florida Healthy Kids, and offers a sliding fee scale.

English, Spanish

Individuals of any age that has any mental health problem
