Description:

Mount Sinai Eldercare PACE is an all inclusive care program providing comprehensive medical and social services to seniors in Miami Dade.

PACE program includes:
Primary Medical Care
Preventive Medical Care
Specialty Medical Care
Rehabilitation and Therapy Care
Social Services
Medication
Home Care
Supportive Services
Nutritional Counseling and Meals
Durable Medical Equipment and Supplies
Transportation
Emergency, Hospitalization, and Nursing Home Care

Intake Process:

Please call for enrollment. A Mount Sinai Eldercare enrollment specialist provides information about the PACE program, assesses eligibility, and invites the potential participant and their family for a tour and meeting with the care team. The specialist assists with financial and medical documentation, coordinates Medicaid eligibility and the Department of Elder Affairs!€™ in-home assessment, and arranges a home visit to evaluate medical and safety needs. After review by the Interdisciplinary Team, eligible individuals are invited to enroll in the program, while those who do not meet criteria receive guidance on appeal options and alternative supports.

Program Fees:

Accepts Medicaid or Medicare or self-pay if not eligible for Medicare or Medicaid.

Languages:

English and Spanish

Provides free language services to people whose primary language is not English, such as:
Qualified interpreters
Information written in other languages

Eligibility:

Be 55 years of age or older
Be certified as eligible for nursing home care by the State Administering Agency
Be able to live safely in the community with Mount Sinai Eldercare!€™s assistance at the time of enrollment
Live in Miami - Dade County
