Intake Process:
To qualify for FACT services, individuals must meet specific clinical criteria, such as:
A history of multiple psychiatric hospitalizations or crisis interventions
Significant difficulties in daily living tasks without substantial assistance
A documented history of co-occurring disorders or criminal justice involvement.
Admission assessment is conducted by Team Lead
