Description:
This service center provides, prescription delivery, laboratory, transportation, and insurance enrollment and:
Primary Care: acute and chronic disease management, preventative care, health education, etc.
Pediatric Services: annual check-ups, acute and episodic care for all illnesses, chronic disease management, school physicals, parent education and immunizations.
OB/GYN: Annual Check-Ups and Pap Smears, family planning and contraceptive management, prenatal care, pre- and post-menopausal treatment, infertility and endocrine treatment, infectious disease diagnosis and treatment, cancer screening and treatment.

8:30am-5:00pm (Mon, Wed, Fri )
8:30am-7:00pm (Tue & Thurs)
Child Immunizations/School Physicals:
8:30am-12:30pm, Monday, Tuesdays , Wed and friday.
Intake Process:
Walk-ins welcome. Appointment recommended.
Child Immunizations:
Must bring immunization records.
Parent or guardian must be present.
Program Fees:
Fees may vary depending on service accepts Medicaid, Medicare and sliding scale available.
Child Immunizations: $5 administration fee for up to 3 shots
School Physical: $40
Sports Physical (no EKG): $40
Sports Physical (with EKG): $50
