Description:
CHI Service Partnership addresses the target population of Richmond/Perrine, and Coconut Grove. The provision of services is offered through a network of partners within the community.
The target population is children/youth and their families that need assistance in reducing chronic absenteeism/truancy, youth violence, and child maltreatment. Other areas of focus are improved school performance, family involvement, and access to healthcare. Families will benefit from Care Coordination in the form of Family Team Conferencing, Case Management, and Outreach.
