General Information

Hours:

8:30am-4:00pm, Mon-Fri

Intake Process:

Must be a patient of New Horizons

Program Fees:

Self-Pay/Sliding Fee Scale, Advantage Plans, Medicare, Medicaid, CMS, MMA and Commercial Plans

Languages:

English, Spanish, Creole

Eligibility:

Patients newborn and over with a history of psychiatric treatment and/or hospitalization.

Adults 18 years or older must fall within Appendix J
Children 6-18 years old must fall within Appendix I

We do not provide services to the follow populations:
Axis I conditions: dementia (organic; non-organic), cognitive disorders, Alzheimer
Axis II conditions: personality disorders
