Description:

FPC, a Program of All-Inclusive Care for the Elderly (PACE), provides managed long-term care and other health-related services to participants in Miami-Dade and Broward counties.

PACE is approved by the Center for Medicare and Medicaid Services (CMS), the Agency for Healthcare Administration (AHCA) and the Department of Elder Affairs for individuals who need long-term care services and who are eligible for Medicaid and Medicare combined, only Medicaid or wish to private pay. FPC provides the opportunity to receive all healthcare services covered by Medicaid and Medicare under one plan with a physician and care team guiding and coordinating your needs across all settings: home, hospital, nursing facility, outpatient departments and day centers. Managed long-term care means that a coordinated plan of care and coordinated services are provided to individuals who choose to enroll in FPC. Your assigned primary care provider along with your care team orders these services.
