Primary Care: acute and chronic disease management, preventative care, health education, Outpatient Mental Health Services etc.

Dental Care: initial exam and diagnosis, preventative care, restoration, surgery, prosthetics, cosmetic surgery, etc.

8:30am-6:00pm, Tue-Fri;
8:00am-12:00pm, Sat

Child Immunizations/School Physicals:
8:30am-12:30pm, Saturday August 5, 2023

Intake Process:

Walk-ins welcomed, appointment recommended.

Child Immunizations:
Must bring immunization records.
Parent or guardian must be present.
Clinic is walk-in (on that Saturday only).

School Physicals:
Must bring child's immunization records for any physical.

Program Fees:

Fees may vary depending on service accepts Medicaid, Medicare and sliding scale available.

Child Immunizations: $5 administration fee for up to 3 immunizations
School Physical: $40
Sports Physical (no EKG): $40
Sports Physical (with EKG): $50

English, Spanish, Creole

Open to the public, insured or uninsured
