This service center provides laboratory, prescription delivery, transportation and insurance enrollment:

Primary Care: acute and chronic disease management, preventative care, health education, etc.

Pediatric Services: annual check-ups, acute and episodic care for all illnesses, chronic disease management, school physicals, parent education and immunizations.

Child Immunizations/School Physicals:
8:30am-12:30pm, Saturday August 5, 2023

Intake Process:

Must call to make an appointment.

Program Fees:

Fees may vary depending on service accepts Medicaid, Medicare and sliding scale available.

Immunizations: $5 administration fee for up to 3 shots
School Physical: $40
School Physical with EKG: $50

Languages:

English, Spanish, Creole

Open to the public, insured or uninsured.
