Description:
In home or center based assessment/case management, weekly or biweekly as needed. Home based or center based assessment of family domains for purposes of family service planning and identification of family strengths and needs. Provides information, resources, and support of families. Working and supporting providers and families to maintain engagement in services and address issues and concerns as they arise. Works with children and victims of domestic violence as well.
In home/on site mental health assessment and consultation on child behavior and parent/child relationship. Consultation is provided to families and/or system provider.
