Description:
The Sweet Life Program was created to target one of the most common chronic diseases seen in the primary care setting ! diabetes. The program is managed by a multidisciplinary team of doctors, dietitians, nurses, healthcare coaches, social workers and exercise physiologists.
Program overview: you must be a primary care patient of Baptist Health Medical Group to enroll in our program. Program is available at different locations must be refer by physician.
*The Program is 6 month long
*Requires to meet once a month
*A 3 hour nutrition class required.
