VA - MEDICAL FOSTER HOME PROGRAM

Description:

The VA Medical Foster Home (MFH) Program offers an affordable, long-term care option, and thus serves as an alternative to a nursing home or institutional placement, typically chosen by Veterans who are unable to live independently due to declines in physical, cognitive or psychosocial functioning.

Medical Foster Homes provide the following:

private rooms
medication management
meal preparation and snacks
housekeeping and laundry
socialization and recreational opportunities

General Information

Hours:

8:30am-4:00pm, Mon- Fri

Intake Process:

To be eligible for a Medical Foster Home, Veterans must be enrolled in Home Based Primary Care, and a Home needs to be available.

A VA social worker or case manager can help you with eligibility guidelines for Home Based Primary Care and Medical Foster Home care.

Program Fees:

$1,500 to $3,000 each month based on your income and the level of care you need.

English. Spanish, Creole

Eligibility:

Veterans who are unable to live independently due to declines in physical, cognitive or psychosocial functioning.
