9:00am-5:00pm, Mon-Fri
Saturdays and Sundays ON CALL
Intake Process:
Must call to complete a free in home assessment for clients.
Program Fees:
All services are paid through Medicaid Title 19 and Title 21 and will not cost parents any out of pocket costs.
Accepts Florida Kidcare, Private pay, Long term care.
Languages:
English, Spanish, Creole
Eligibility:
All ages with a chronic physical, developmental or emotional condition.
Client must have Medicaid and services must be medically necessary and ordered by a physician.
