Intake Process:

Digital registration documents are available for you to complete in your client portal. To register, you will need three items:

Wi-Fi connected device,
a front/back copy of your insurance card and
a method of payment. The linked credit card must be valid at the time services are rendered !€“ in the future.

We will automatically be notified when your registration documents have been submitted.

Next, we will verify your account information and then we will email you to schedule an appointment. If you need help creating your client portal, please do not hesitate to reply to this email and a member of our support team will assist you further.
