Description:
Strengthens families' self sufficiency by identifying needs and referring them to appropriate service providers. The program develops a case plan on family concerns and provides a 90 day follow up on progress.
Services are provided in the following manner:
1. Intake & assessment
2. Referrals made based on family needs
3. 6 month program services

9:00am-5:00pm, Mon-Fri
Intake Process:
Referral or walk-in into the program, where an intake assessment case plan is developed and referrals made out to service agencies based on the client's needs. Referrals are accepted from DCF, DJJ, schools, community-based agencies and walk-ins.
