As part of our commitment to comprehensive care, Centrum Health has partnered with the Florida Department of Children & Families (FL DCF) as a Community Access Partner. Through this collaboration, Centrum Health assists individuals in applying for essential public assistance programs, including:

Supplemental Nutrition Assistance Program (SNAP)
Temporary Assistance for Needy Families (TANF)
Medicaid

With 11 primary care clinics in Miami-Dade County to provide medical assistance for those eligible for Medicaid, Medicare, and Market place (ACA).

Intake Process:

Intake Scheduling: Prospective patients can schedule appointments by calling 888-726-5116 or visiting the Centrum Health website.
In-Person Assistance: For those preferring in-person assistance, staff at any Centrum Health location can help with the intake process.
