CONCERNED AFRICAN WOMEN - SYNERGY SERVICE PARTNERSHIP

Description:

The Synergy Service Partnership will provide care coordination services, using the evidence based model, Family Team Conference targeting youth, parents, families who reside in the Miami Gardens Community linking them to community resources that would prevent chronic absenteeism, truancy, and child maltreatment; increase parental involvement; and decrease youth violence.

9:00am-5:00pm, Mon-Fri;
Hours may vary, call for more information.

Intake Process:

Participants will go through an eligibility screening and those determined to be eligible for care coordination will receive an initial assessment to confirm eligibility. The Assessment will define family strengths and services needed.
