CITRUS HEALTH NETWORK - PERMANENT SUPPORTIVE HOUSING

Description:

Provides supportive housing services through a variety of programs for individuals and families with a disability, veterans, and for the chronically homeless. Clients are provided with housing in apartments at specific sites or with rental assistance in an apartment of their choice in the community. The goal is for the client to achieve independent living through stable housing and additional individualized support services such as case management, psychiatric treatment, and primary care services.

9:00am-5:00pm, Mon-Fri

Intake Process:

Call for an appointment and more information

Veterans must bring ID, housing prices and to apply you must be in a housing Crisis such as:

* Homeless
* 3 date eviction notice from the court or a vacant letter
* Domestic violence victim with a police report
* Section 8

Program Fees:

Depending on client's income, may pay up to 30% of rent

Languages:

English, Spanish, Creole (limited)

Eligibility:

Individuals 18 year or older with a history of psychiatric disability, veterans, and chronically homeless and in need of permanent housing
