Description:

Open Door Health Center is a primary healthcare center dedicated to serving the uninsured of South Dade at no cost to them.

The services provided include:
Adult care
Women's care
Pediatric care
Enhance Fitness Program
Women's Health Program
The Prescription for Health Diabetes Program
Diagnostic services such as lab work, x-rays, pap smears, and mammograms are available
Educational Services for adults, children, and teens
Treatments such as medications for chronic illnesses and immunizations are also available
Support groups for people with Type 2 Diabetes and their relatives are open to the community
Secondary healthcare by community volunteer specialist physicians

9:00am-8:00pm, Tue;
9:00am-5:00pm, Wed-Fri;
8:00am-12:00pm, Sat

(Closed 12:00pm-1:00pm daily for lunch)

Intake Process:

Complete an application obtained at the Open Door Health Center or online.

Must meet with a representative from the Human Services Coalition, Clinic Health Navigator or Registration Specialist located on site who may assist the individuals in applying for other services they may qualify for.

Must show proof of income.

Application is processed in about 2 weeks - 1 month depending on client meeting eligibility requirements.

Eligibility:

Must be uninsured, have an income 200% below federal poverty guidelines, and be a Miami Dade County resident.
