Intake Process:

To apply for the Patriot Passport eligible veterans must complete the Patriot Passport EASY Card Application found online.

Service Connected Disabled veterans need to provide:

DD214 form showing proof of an honorable discharge.
A VA disability letter showing percentage of disability.
A Valid state-issued Florida Driver!€™s License or Florida ID showing a Miami-Dade County physical address.
Social Security Card

Other veterans need to provide:

Proof of Income - Consecutive pay stubs, weekly (4-pay stubs), bi-weekly (2-pay stubs) if paid monthly (1-pay stub) or current income tax return or IRS Tax Return Transcript.
Income Tax W2 wages statement.
Current valid Florida ID or driver's license with a Miami-Dade County physical address.
DD214 form as proof of an honorable discharge.
